Broadly speaking there are instant-result home testing and laboratory tests - both use different methods. Regarding point of care / home testing most manufacturers report >95% accuracy, however there was hesitation to them being widely rolled out. This comes down to 2 main reasons; False negatives - most tests have been validated on known COVID-19 patients that have been admitted into hospital. By default these patients will be quite unwell and have likely mounted a strong immune response so antibodies will be more easily detected. In the community however some patients have mild symptoms or are even asymptomatic. These patients may produce lower antibody levels which may not be detectable by current point pf care tests. False positives - there are other coronaviruses which have been circulating seasonally for many years - often causing mild cold / flu like symptoms. We are unsure whether these home testing kits are specific enough. So an individual may test as positive if they have been infected with a coronavirus that is not SARS-CoV-2, this is obviously a concern and can lead to individuals / families / companies making unwise decisions based on the assumption that they are immune.  New Laboratory testing using methods such as ELISA is more promising. They can provide quantitive antibody levels, rather than a negative or positive reading, so are able to identify those patients that had mild symptoms with lower antibody levels in the community. The body produces different antibodies to different parts of the SARS-CoV-2 virus. Some antibody responses are stronger than others. The better tests now look for more than 1 antibody which also increases sensitivity. Also many of the lab based tests report better no cross reactivity to other coronaviruses which increases specificity, this should mean less false positives. In fact, the latest laboratory based antibody tests have sensitivity and specificity values of over 99%. The big question however remains, what does a positive antibody result mean? Does is guarantee protection from COVID19 or a less severe infection in the future - hopefully yes, but unfortunately we don't yet know for sure. For the time being we need to interpret with caution and those testing positive for antibodies still need to take precautions.  ### COVID 19 Adapting Practice and Remote Monitoring The last few months have brought significant changes to the healthcare landscape across all sectors. In primary care there has been an overnight change in practice with almost exclusive reliance on telehealth and remote consultations via video or phone. Pulse Oximetry in the fight against COVID-19 is proving vital. These relatively simple medical devices provide valuable info when assessing patients with many respiratory conditions - especially COVID-19. Given the importance of the data - our practice has been sending them out to our members for no charge. Likewise, local NHS services have started to provide remote pulse oximetry for suspected COVID-19 cases. In conditions like asthma and pneumonia, patients are usually significantly short of breath before oxygen levels start to fall. However, with COVID-19 we sometimes see oxygen levels drop before shortness of breath develops - termed silent hypoxia - for patients in the community it may be an early warning sign. Similarly, some physicians stress patients by getting them to exercise whilst monitoring oxygen saturations. A significant drop in oxygen levels can indicate serious underlying lung pathology and can be used to identify those patients who need hospital admission and close monitoring (not something I would recommend doing remotely).  I've remotely consulted with patients with COVID-19 in the NHS and private sector who were not short of breath but during regular follow up their oxygen levels were found to be low. They were admitted into hospital and treated with oxygen (did not require ventilation), made a good recovery and discharged after a few days. Picking up low oxygen levels early into the illness may result in better outcomes. A pulse oximeter, together with a thermometer and blood pressure machine provide information that can potentially influence decision making in a particular case. It's not to say that 02 levels are the only factor, those with normal levels may also require hospital admission. A full clinical assessment is always necessary, but pulse oximeters provide a key piece of data. ### Workplace Support in the COVID-19 era - Back to Work Strategies Just as we have a core value to treat individual patients in a personalised and bespoke manner, we have exactly the same ethosmanaging our corporate clients.  Up until now our main work with small to medium sized businesses was looking after executives as part of our Concierge Clinic membership, providing both acute and chronic medical care with a strong emphasis on bespoke health screening and preventative medicine in order to optimise health.  However, in view of the recent coronavirus outbreak we have been asked to work with businesses to advise on safely gettingback to work. Though each business is different with specific requirements and solutions we focus on four main areas.  - Reducing risk of exposure this can include strategies such as working remotely from home, having less people in the office with physical distancing measures and adequate office ventilation. It's also important to look at the way individuals travel into work and the risk that that entails. In addition, there have been discussions around wearing of masks particularly in indoor working environments and daily temperature monitoring.  - Containment involves each individual reporting any symptoms, no matter how mild, so that their case can be assessed and they can be advised to self-isolate as required. This can also extend to reporting symptoms of anyone who is unwell in the household. With the use of healthcaretechnology, we now have clients who monitor vitals such as temperature, pulse rate and blood pressure at home daily prior to going to work. These readings get automatically sent via an app to the physician who can assess real time data. Setting up alerts on the app so that temperatures above 37.5 result in an urgent push notification to the physician means the case can be assessed urgently.  - Testing there are various ways of doing this and although there are limitations to all current testing methods, some organisations have requested antibody testing to assess whether employees / executives have previously been infected with COVID19. However, it's important to note that even if this is the case, we are not yet sure whether this means they will not pick up the infection again. There are multitudes of antibody tests available with varying degrees of accuracy. The point of care test kits, although providing an instant result, is not specific or sensitive enough in the community for it to be reliedupon. The laboratory tests such as Abbott's antibody test is significantly more accurate with more than 99% sensitivity and specificity.  In addition to antibody testing there are also options for PCR testing (which tests for the actual virus) being trialled by organisations. This is usually recommended for workers who are symptomatic or if a member of their household is symptomatic. We still need to be cautious with negative tests as PCR swabs do have a false negative rate. Combining testing of SARS-CoV-2 with other respiratory viruses can give us further information. Notably, if another respiratory virus is showing as positive then we are less worried about a negative COVID-19 swab being a false negative (in saying that sometimes coexisting infections can also be present). Some organisations prefer to test their employees on a regular basis even if they are not showing symptoms to pick up asymptomatic carriers and those in the incubation period. Whilst this is possible, there is not any evidenced based guidelines into how effective this strategy is and how frequently testing should occur.  - Personal Risk Stratificationcan be overlooked but is certainly a vital part of any back to work strategy. Patients that fall under the shielding category need to continue following the relevant Public Health England guidance. However, those with medical conditions and other risk factors falling into the moderate risk group also need to take extra precautions. The challenge is the moderate risk group covers a very broad spectrum of risk and a one size fits all approach is simply not safe. Calculating an individual’srisk / probability of developing COVID-19 complications is important before deciding on further steps. There are various risk calculations tools available and these should be used to guide decisions but it’s also important to note many risk calculators are not validated and they will likely become more accurate and sophisticated as further evidence becomes available  -continuous review is important. In summary, during what is likely to be an uncertain few months ahead of us, a bespoke and flexible plan is required for most businesses to get safely back to work. It's important that any plan is regularly reviewed, particularly in light of any new developments as the pandemic progresses. ### Covid19 Testing Overview Testing is important but many are falsely reassured after a negative test. An area of concern is that individuals are being tested without clinical input and an explanation of the relevant test and limitations. Here's a summary of the 2 main tests available: PCR tests - throat / nose swab that test for presence of the actual virus. They are usually accurate when you first start developing symptoms (likely withing the first 7 days) but this is dependent on taking the swab correctly. An incorrect swabbing technique can certainly give a false negative result.  This test can also be positive in those with no symptoms - so called asymptomatic carriers who are still able to spread the virus.  The PCR swab may show as positive in the incubation period, possibly 1 or 2 days before developing symptoms in some cases but this is not completely reliable and therefore despite a negative test you may still be a carrier and able to infect others.  We are unsure how long the test will remain positive for, as once symptoms develop individuals will clear the virus at different rates. Another interesting development is that some people test positive even 4-5 weeks after becoming symptomatic - currently the consensus is they are not infectious at this stage but the swab is being overly sensitive and picking up inactive viral genetic material. Serology - generally a blood test which looks at the immune response specifically to the SARS-CoV2 virus. Serological tests can look for for both short term (IgM) and long-term (IgG) antibodies. In essence it looks for previous COVID19 infection but is not ideal for assessing acute current symptoms.  Timing of the test is important. The antibodies may not show up as positive in the first few days of having the illness so if tested too early you may get a false result. Most manufacturers state tests are accurate after day 7 - 14 of having symptoms.  The IgG antibody is likely to remain positive for at least a few months. It can show if you had COVID19 in the past but we are unsure if this means future protection and immunity. As with most tests there are also limitations (discussed in next update) - this is especially the case with point of care / home tests. ### General Health & COVID-19 In a world where there seems to be heightened anxiety and panic, we must focus on what we can control. There is emerging information regarding risk factors - some non-modifiable such as age, ethnicity and male gender. However, in terms of modifiable risk factors, obesity, cardiovascular disease, diabetes, hypertension and lung disease infer an increased risk. The anxiety around coronavirus is partly due to the uncertainty. Hand washing and maintaining distance from those with flu like symptoms in addition to wider social distancing measures will help reduce infection rates. In addition, although the evidence about wearing masks is not conclusive there appears to be some benefit and many countries have adopted rules for wearing masks in public.  But there's more we can all do to get our immune systems primed to give us the best chance of fighting infections and a sense of some control. In general:  - getting enough sleep (there are many entrepreneurs that live on 4 hours sleep, which may not cause too much of an issue in the short term but it's not a good sustainable long term plan!) -  moderate exercise for approximately 20 mins most days (over exercising can have a negative impact) - Eating well (whole grains, nuts, seeds, fruit, vegetables) - Avoiding excessive alcohol - Avoiding smoking - although there are some reports that nicotine may have a protective effect, the other chemicals present in cigarettes damage the lungs and put you at risk of a more severe infection. On a more personal level for those with other health conditions it's important to optimise the numbers. A preventative and proactive approach to healthcare is vital.  Focus on getting parameters such as HbA1c (average sugar levels), cholesterol, blood pressure and weight to target. This can be done through a combination of lifestyle measures and / or medication.  Optimising medications for other conditions such as asthma and COPD are also important. There's not enough evidence yet to say that this will translate in fewer complications / deaths, however it will improve overall health and logically that should lead to fewer complications. ### Female Health Screening & Preventative Medicine Health screening plays a vital role when looking at the wellbeing of women, especially after the age of 40, says Dr Amarjit Raindi, a GP at 25 Harley Stree... view more ### Vomiting in Pregnancy Hyperemesis gravidarum is more likely to occur if you are having a multiple pregnancy... view more ### The Best Concierge Doctor Service in London Part of being a private concierge doctor means you have to be prepared for any possibility – no two days are ever the same, keeping the work diverse, fascinating and interesting. Although there is no sinister underlying disease process it can cause a multitude of symptoms including abdominal pain, bloating and altered bowel habit. Some patients have particularly severe symptoms leading to a significant impact on quality of life. Diet plays an important role in IBS and below we discuss ways in which you can altered your diet to improve symptoms. There is no single diet that will help all patients with IBS, advice is usually tailored to your symptoms and triggers. Routine, Routine, Routine... We all generally function better when we have a set routine and your bowels are no different – especially when you have IBS. It’s important to eat your meals at approximately the same time each day – this includes weekends! Slowing down is also important. Many of us have busy schedules and often meal times are rushed. This tends to cause more stress on the bowel and aggravates IBS symptoms. Take enough time with meals, chew properly and don’t rush. Drinking Maintaining good hydration levels is important with IBS – ensure you are drinking at least 2 litres per day of mainly water (unless you have any other medical condition which requires you to restrict your fluid intake). In patients that are constipated the fluid works with fibre in the diet to help relieve constipation. In individuals who have diarrhoea predominant IBS the fluid intake is important in recouping the fluid loss through loose motions. Caffeine should be limited to no more than 2-3 cups of tea / coffee per day. This is due to caffeine stimulating bowel movements which can worsen symptoms. Furthermore, alcohol and carbonated drinks should also be minimised as both can worsen symptoms of diarrhoea. Low sugar drinks and those with added sweetener should also be avoided as they contain sorbitol which can lead to bloating and diarrhoea. Probiotics Probiotics are now readily available and can come in the form of capsules, drinks and yoghurts. They help to replace the “bad” bacteria in our bowels with “good” bacteria. Some studies have shown an improvement in IBS symptoms (abdominal pain and bloating) when using probiotics, however they do not seem to work for everyone. The consensus is to try a particular probiotic for at least 4 weeks to assess response. Not all fibre is equal We are often told that having fibre is an important aspect of our diet but in patients with IBS it can exacerbate symptoms. There are two main types of fibre – soluble and insoluble. In patients with diarrhoea predominant IBS it is advisable to reduce insoluble fibre. This type of fibre does not easily dissolve in water and is found in whole-wheat bread, brown rice, and cereals. It adds more bulk to the stools which can make bloating and diarrhoea symptoms worse. Soluble fibre is found in oats, barley, fruit and vegetables. It dissolves in water and forms a soft gel which can help aid the passage of stool. Increasing soluble fibre can help relieve symptoms in patients with constipation predominant IBS. What are FODMAPs? FODMAP stands for fermentable oligo-saccharides, di-saccharides, mono-saccharides and polyols. These are essentially carbohydrates that are not well digested and enter the large bowel. Once there they draw in water in the large bowel and are fermented by gut bacteria leading to production of excess gases. This in turn can cause bloating and abdominal pain. Having a low FODMAP diet has been shown to improve IBS symptoms in up to 80 % of patients. Examples of foods that are high in FODMAPs are numerous, including apples, pears, sweeteners, milk, cheese and cabbage. It is important to attempt a low FODMAP diet in conjunction with your doctor and a dietician. For more information or a comprehensive consultation with a bespoke management plan please contact us at the GP Clinic London. ### Irritable Bowel Syndrome (IBS) What is IBS? Irritable bowel syndrome (IBS) is a collection of gut symptoms that occurs in the absence of any other pathological cause. It is a common condition affecting up to 20% of the population and usually first affects peoples in their 20’s. It can often cause distressing symptoms and have a significant impact on quality of life. We are not sure of what causes IBS but in some patients it seems to be triggered after an episode of gastroenteritis. Do I have IBS? IBS symptoms include the following: Bloating Abdominal pain Diarrhoea Constipation Urgency to open bowels Feeling of incomplete bowel emptying The symptoms need to be present for at least 6 months in order to positively diagnose IBS. Stress and anxiety can trigger the symptoms or make them worse. Typically, the bloating and abdominal pain is worse after eating and better after opening the bowels. The bowel habits may be altered in different ways, some patients have diarrhoea, others constipation and some with a mixed / alternating pattern. Other Causes Seeing a doctor is important if you feel you may have IBS – the symptoms are non-specific and other conditions can cause similar symptoms. It is important for a clinician to take a thorough history and examination to rule out inflammatory bowel disease (ulcerative colitis and Crohn’s) and coeliac disease. Other less frequently encountered conditions also need to be excluded depending on risk factors, age and symptoms. For instance, bowel cancer may present with a change in bowel habit and ovarian cancer can present with symptoms of bloating. Blood tests are usually recommended as a first line investigation, but other tests may be required such as stool tests, abdominal / pelvis ultrasound and colonoscopy. It is only if these tests come back as negative that a diagnosis of IBS can be made. Red flags If you have any of the following symptoms it is worth seeing a doctor as soon as possible to rule out any other causes. Sometimes these symptoms can be associated with potentially sinister conditions: Blood in the stools Weight loss New onset symptoms after the age of 40 What Treatment is available? It is important to tackle IBS symptoms with a holistic approach. We know that stress is linked closely with IBS symptoms so this is an important area to focus on. Exercise and relaxation practices such as yoga and meditation can be hugely beneficial. There is also some evidence that acupuncture can help. In some cases, psychological therapies like CBT (cognitive behavioural therapy) and hypnotherapy have been shown to be effective. Diet is an important aspect of IBS management. Ensuring you eat meals at approximately the same time each day and at regular intervals can help with symptoms. Also maintaining good hydration throughout the day and cutting down on tea, coffee and alcohol can help. Specific dietary changes can also result in improved symptoms – this includes changing the type of fibre that you consume and low FODMAP diets – see article “5 dietary changes for IBS patients” for more information. There are different types of medication that can help: For abdominal pain and bloating – antispasmodic medications that relax the bowel (such as hyoscine and mebeverine) can be helpful. Also peppermint oil capsules can significantly reduce bloating. In patients with diarrhoea predominant symptoms the use of loperamide can settle loose stools. Bulk-forming laxatives can be used in constipation predominant IBS. Second line treatments include the use of antidepressant medications (both tricyclic antidepressants and selective serotonin reuptake inhibitors) – they are used in lower starting doses than those used for depression. These medications require monitoring and follow up and can often result in a significant reduction in symptoms. There are over 30 different conditions that are tested for including • heart disease • Alzheimer's disease • osteoporosis • diabetes • cancers (including breast, colorectal, lung, prostate and skin) This test also evaluates your lifetime risk of developing these conditions and compares it to the general population. In addition to genetic risk, the diseases we test for are also influenced by environmental and lifestyle factors. If we identify an individual with a high genetic risk of developing a certain disease we are able to offer a personalised management plan which could include medication, surgery and lifestyle changes in order to reduce the risk of developing the condition. The Genetic nutrition and fitness test is a one-off test which looks at your genetic ability to lose weight and response to different types of exercise such as cardio and weight training. It also looks at your risk of developing certain vitamin and nutrient deficiencies as well as your genetic ability to metabolise fat, protein and carbohydrates. This information can be very useful when setting up a tailored personal exercise and nutrition program.   Book Appointment Online 0207 118 1816 8.30AM to 6PM 07863 112 919 Evenings and Weekends info@thegpcliniclondon-com.stackstaging.com ### High Profile At The GP Clinic London we are experienced in treating high profile & prominent individuals and families. The way that our bodies react to medication is partly explained by our genetic make up. We can now study our individual genes and this can tell us: - the drugs which are most effective for us as individuals when treating certain medical conditions - those medications that are more likely to give us adverse effects and side effects. It takes out some of the guess work and trial and error involved in prescribing. Examples of where this testing is particularly useful includes the prescribing of pain medication, psychiatric medication (including depression and anxiety) and cardiovascular conditions such as high blood pressure and cholesterol.   Book Appointment Online 0207 118 1816 8.30AM to 6PM 07863 112 919 Evenings and Weekends info@thegpcliniclondon-com.stackstaging.com ### Biological Age / Telomere Testing Ageing is an independent risk factor for many different diseases including cancers, ischaemic heart disease and diabetes. An individual's rate of ageing will vary according to: • genetics • environment • lifestyle With telomere testing we can now assess an individual's biological age and consequently their susceptibility to certain diseases. Telomeres are the protective caps on the ends of chromosomes and as all cells replicate and age, the telomeres get shorter. Cells with short telomeres lose function and die - this is an essential part of the ageing process on a cellular level. The length of telomeres (and the ageing process) can be altered by lifestyle changes such as exercise, stopping smoking, reducing alcohol, changes in nutrition, reducing stress and improving sleep. Telomere testing has the following applications: • identify a patient's biological age • monitor the ageing process • analysing biological age alongside other health screening tests to assess current health status Essentially by identifying an individual showing accelerated ageing we can tailor a management programme to slow or reverse telomere loss and consequently slow down the ageing process and reduce risk of developing age-related disease. This test also indicates susceptibility to certain age related diseases. It is best repeated on annual basis to monitor how lifestyle is having an impact i.e accelerating or delaying the ageing process. ageing process. Genetic Predisposition Test A one off screen which calculates your risk of developing certain conditions such as cancers, heart disease, alzheimer's, diabetes. 34 different diseases are tested. Hereditary Cancer Test This test looks for mutations in 98 genes associated with 25 hereditary cancers – including prostate, colon, gastric, ovarian and breast cancer. It is a more detailed look at cancers compared to that available in the genetic predisposition test. Genetic Nutrition and Fitness Test A one off tests which looks at genetic ability to lose weight, genetic risk of developing certain vitamin deficiencies, genetic response to exercise (cardio and weight training), your genetic ability to metabolise fat, protein and carbohydrates. This test can help guide your diet and exercise regime for optimum results. Pharmacogenomic Test For truly personalised medicine – this test looks at how effective certain medications are for you and which are more likely to give you side effects. It effectively takes out some of the guesswork involved in prescribing medication. It looks at many different classes of medication including psychiatric (anti-depressants), cardiovascular (cholesterol, blood pressure), endocrine (diabetes), neurological, and pain medication. It is highly infectious and in most cases a mild and self-limiting illness. Most patients have mild features of systemic upset such as fever and lethargy. The typical chickenpox rash is characterised by red spots and vesicles (fluid filled spots), which can be intensely itchy. The spots usually start to scab over after 5-7 days. Uncommon complications of chickenpox include ear infections, pneumonia and encephalitis. Certain at-risk groups can be more prone to complications and have a more severe course. These include immunocompromised and elderly patients. Contracting chickenpox in pregnancy can also have harmful effects to the fetus. A chickenpox vaccine is available (not part of the standard immunisation schedule in the UK). This is given as 2 doses which should be 4 -8 weeks apart. The vaccine may be considered for those that have not had chickenpox before, especially if they are in close contact with people that are immunocompromised (e.g family members undergoing chemotherapy) or adults that are working in healthcare. Key Points: • 2 doses required • Cost per dose £70 • Interaction with other vaccines: Has to be given on the same day as the MMR vaccine or at least 4 weeks after Please contact us for further information. It is transferred to humans by the saliva of infected animals – through a bite or scratch. It is carried by mammals including dogs, primates and bats. Symptoms can develop several months or even years after a bite, they include fever, headache and malaise. This can progress to confusion, anxiety, hallucinations and muscle weakness. Virtually all cases are fatal once symptoms develop. Avoiding contact with animals in high risk areas is important to minimise the risk of contracting rabies. There is also a vaccine available which required 3 doses to be administered over 3 - 4 weeks. In the event of a bite (regardless of whether you have been vaccinated) clean the wound promptly with soap and water for several minutes and use a disinfectant. Seek medical review urgently. Further treatment will be offered depending on your vaccination status. Unvaccinated patients will require rabies immunoglobulins (this can be difficult to obtain in certain countries / locations) and 5 doses of rabies vaccinations. Those that have completed a full vaccination course will require 2 further doses of rabies vaccine. Key Points: • 3 doses required over 3-4 weeks • Cost of Rabies vaccine £65 per dose Please contact us for further information. Book Appointment Online 0207 118 1816 8.30AM to 6PM 07863 112 919 Evenings and Weekends info@thegpcliniclondon-com.stackstaging.com ### Cholera Cholera is a bacterial infection which affects the gastrointestinal system. It is more common in areas where there is poor access to clean water and inadequate sanitation. It is contracted by consuming contaminated food or water and can also be passed on from person to person leading to outbreaks. Cholera is a potentially fatal infection – it can lead to watery diarrhea, dehydration and sepsis. It is usually treated by rehydration and in some cases antibiotics may be required. Some patients need to be admitted into hospital for intravenous fluids. Taking precautions such as drinking bottled or cooled boiled water are important. In addition, there is also an oral vaccine which protects against cholera and is recommended depending on where you are travelling. Key Points: • Oral vaccine, available over the age of 2 • 2 or 3 doses depending on age, given between 1 – 6 weeks apart • Can be given with other vaccines Please contact us for further information. Book Appointment Online 0207 118 1816 8.30AM to 6PM 07863 112 919 Evenings and Weekends info@thegpcliniclondon-com.stackstaging.com ### HPV Vaccine in Males Currently in the UK the HPV vaccine is part of the immunisation programme for females. However, it is also suitable for male patients. For certain groups such as men who have sex with men (MSM) there is little benefit received from the female vaccination programme. In males, HPV is associated with a significant proportion of anal cancers (up to 80%), approximately half of all penile cancers and a third of oropharyngeal cancers. MSM are at a higher risk of contracting HPV and consequently developing the related cancers listed above. Males between the ages of 9 and 26 can be vaccinated using Gardasil 9, which protects against 9 different strains of HPV. The vaccine is more effective when it is administered before any HPV exposure. It may also be recommended to males (especially MSM) over the age of 26 if they are at continued high risk. Key points: • The vaccines can be administered as a 2 or 3 dose schedule dependent on patient age • Cost of Gardasil 9 per dose £185 Please contact us for further information. Book Appointment Online 0207 118 1816 8.30AM to 6PM 07863 112 919 Evenings and Weekends info@thegpcliniclondon-com.stackstaging.com ### Meningitis Meningitis is a condition which can be caused by a variety of different organisms leading to an infection of the membranes that line the brain and spinal canal. It is a potentially serious and sometimes fatal condition. Symptoms include fever, headache, rash (non blanching), photophobia (light sensitivity), neck stiffness, nausea and vomiting. You can reduce your chance of contracting certain bacterial strains of meningitis by getting vaccinated - there are 3 different vaccines currently available: Meningitis B - this has recently been incorporated into the routine immunisation schedule in the UK. It is a potentially life threatening infection and accounts for the vast majority of meningitis infections in children. Children born after the 1st of May 2015 are eligible for the vaccine on the NHS. 3 doses are given at 2 months, 4 months and 1 years old. If your child was born before 1st May 2015 you can still get them vaccinated in a private clinic. Meningitis C / HIB B - this is a combination vaccine that protects against the meningitis C strain and haemophilus influenzae type B. This vaccine is given as part of the routine UK immunisation schedule at 1 years old. Meningitis ACWY - this vaccine protects against 4 different strains of bacterial meningitis. As part of the UK immunisation schedule it is now offered to school children aged 13 and to first time university students. It may also be recommended to patients before travelling depending on location. For patients travelling to Saudi Arabia for Hajj or Umrah pilgrimage, it is a requirement to have received the meningitis ACWY vaccine within the last 8 years and have certification of this prior to travel. It is recommended to get vaccinated 2 weeks prior to travel. Key Points: • Can be given with other vaccines • Cost of Vaccination and certificate £65 Please contact us for further information. Book Appointment Online 0207 118 1816 8.30AM to 6PM 07863 112 919 Evenings and Weekends info@thegpcliniclondon-com.stackstaging.com ### Tetanus This bacterial infection is a rare but potentially fatal and is present throughout the world. It can enter the body through breaks in the skin caused by cuts, burns or bites. A deep wound containing dirt or soil increases the risk of contracting tetanus. Muscle spasms, pain and fever are some of the common symptoms that can occur. Tetanus vaccination is part of the routine immunisation schedule in the UK. Doses are given at 2,3 and 4 months old. This is followed by a preschool booster at 3 years old and then another aged 14. Further booster doses may be recommended if you have not had one in the past 10 years. It is advisable to seek medical review if you get injured and receive a deep or potentially contaminated wound. Key points: • At least 2 weeks before travel • Combination vaccine also protects against diphtheria and polio • Booster is required after 10 years • Cost of booster £35 Please contact us for further information. Book Appointment Online 0207 118 1816 8.30AM to 6PM 07863 112 919 Evenings and Weekends info@thegpcliniclondon-com.stackstaging.com ### Typhoid Typhoid is a bacterial infection which can occur after ingesting contaminated food. It is more common in certain parts of the world, especially where there may not be easy access to clean water. Symptoms of infection include abdominal pain / cramps, diarrhoea and fever. Typhoid is usually treated with antibiotics and rehydrating. Without the correct treatment the disease can be life threatening. Prevention is the best strategy against Typhoid. Ensure you only drink bottled or boiled water. In addition, vaccination can significantly lower your risk. This can be given as either oral capsules (3 doses) or an injection. It is recommended you get vaccinated at least 2 weeks before travelling. Key points: • At least 2 weeks before travel • Booster required after 3 years • Cost for single vaccination £50 • Cost for combination vaccine with hepatitis A £110 Please contact us for further information. Book Appointment Online 0207 118 1816 8.30AM to 6PM 07863 112 919 Evenings and Weekends info@thegpcliniclondon-com.stackstaging.com ### HPV The human papilloma viruses (HPV) are a family that are responsible for many skin lesions – such as plain warts, verrucas and genital warts. However, they are also responsible for almost all cases of cervical cancer and are a cause of vulval and anal cancers. HPV is relatively common and can be spread through sexual contact. There are some strains of HPV (6, 11, 16 and 18) that can potentially cause changes in the cervix. These changes are usually pre-cancerous in nature and therefore early warning signs. By undertaking regular smears these pre-cancerous changes can be detected and treatment provided so cervical cancer does not develop. The cervical smear screening programme in the UK has been hugely successful in preventing and reducing cervical cancer rates. In addition to the screening we now also have vaccines that can immunise against HPV. There are 2 main types of vaccine. Cervarix protects against 2 strains of HPV (16 and 18) which are responsible for approximately 70% of cervical cancers, whereas Gardasil protects against the 4 most common strains likely to cause sinister cervical cell changes (6, 11, 16 and 18). There is now a new vaccine available (Gardasil 9) that protects against 9 different HPV strains that can potentially cause cervical, vulval, anal and penile cancer. Gardasil 9 can be given to both males and females after the age of 9 until 26 years old. In addition to protecting against cervical cancer the vaccines can also help protect against vulval cancer, anal cancer and genital warts. Patients older than 26 can also be given the vaccine off licence, this is usually recommended if they are considered to be in a high risk group. Prior to any vaccine being administered there would be a full assessment and explanation of the benefits and risks. Key points: • The vaccines can be administered as a 2 or 3 dose schedule dependent on patient age • Cost of Gardasil 9 per dose £185 Please contact us for further information. It is contracted through contact with bodily fluids through sexual intercourse, blood transfusions, tattoos and sharing needles. It can cause symptoms such as jaundice, nausea, vomiting, fever, abdominal pain and diarrhoea. However in some cases it can be asymptomatic. Hepatitis B can also cause a chronic infection and lead to long term damage of the liver – causing liver cirrhosis, failure and even cancer. Vaccination is usually given by way of 3 doses. A blood test can be performed after vaccination to ensure it has worked. Key Points: • 3 doses usually given over 6 months. Accelerated courses of 3 weeks are available • Cost per dose £50 • Also available in combination with hepatitis A vaccine Please contact us for further information. Book Appointment Online 0207 118 1816 8.30AM to 6PM 07863 112 919 Evenings and Weekends info@thegpcliniclondon-com.stackstaging.com ### Hepatitis A Hepatitis A is a viral infection that affects the liver. In most cases the condition is self-limiting. It can be passed on through consuming contaminated food / water and poor hygiene. It can also be contracted through sexual activity. Recently there has been an increase in prevalence in the men who have sex with men group. Symptoms include fever, diarrhoea, vomiting and jaundice. In most patients the symptoms resolve after a few weeks. However, in a very small percentage of cases the illness can be more severe and even fatal. Making sure you are immunised against hepatitis A can reduce the chance of contracting the illness significantly. Usually 1 injection is given followed by another 6-12 months after for long term immunity. It is recommended you get vaccinated at least 2 weeks before traveling. Key points: • At least 2 weeks before travel • Booster 6-12 months after for long term immunity (20 years) • Cost for single vaccination £50 • Cost for combination vaccine with typhoid £110 Please contact us for further information. The virus attacks and weakens the immune system (CD4 T cells) which eventually leads to AIDS where the body is susceptible to infections that would not normally cause an issue in a healthy individual. Patients that contract HIV can sometimes present with a seroconversion illness - this is a flu-like illness which can present up to 6 weeks after exposure. In some patients the only symptom may be a generalised and persistent increase in the size of glands (lymph nodes). However, in many patients there may be no symptoms for many years. Developing AIDS is not inevitable - the advances in HIV treatment mean that if diagnosed and treated early enough the life expectancy is similar to that of an individual without HIV. This has been a huge advancement in recent years and emphasises the importance of early testing. Testing for HIV can be done as early as 10 days after exposure by looking for the genetic code of the virus itself (HIV RNA), or after 28 days by testing for HIV antibodies (the immune system response after HIV infection) and a specific HIV protein (p24 antigen). A post exposure prophylaxis (PEP) can be offered to patients that have had high risk exposure - this involves taking anti-HIV medication for 28 days (consisting of 3 active drugs) and reduces the chance of developing HIV. It must be taken within 72 hours of high risk exposure (the earlier the better) for it to be effective. The medication does have side effects which can result in patient's not completing the course. Pre-exposure prophylaxis (PrEP) is an option for HIV negative patients that are at high risk of contracting HIV (e.g for individuals whose partner is HIV positive, men who have sex with men). This involves taking an anti-HIV medication (Truvada) on a daily basis or a short regime during the time of intercourse. Studies have shown PrEP greatly reduces the transmission of HIV. The herpes simplex virus type 2 (HSV2) is most commonly responsible for genital herpes infection, HSV1 can also cause genital infection but is more commonly associated with oral infections. There has been an increase in the prevalence of genital herpes especially in the men who have sex with men group. The infection can be passed on when having intercourse with a partner who has active symptoms and also in those that have had previous infection but are currently asymptomatic. Furthermore, a person with herpes may not have ever had any symptoms and can pass this on without knowing. Multiple partners and intercourse without condoms are also risk factors. Symptoms include a painful eruption of sores or blisters, pain on passing urine and vaginal / penile discharge. The virus stays latent in the body and can be reactivated to cause subsequent infection with similar lesions which can take up to 10 days to resolve. Treatment is with antivirals – if initiated early enough can cause a reduction in the severity of symptoms and reduce duration. In individuals having frequent attacks a long term daily course of low dose antivirals may be recommended to reduce the chance of an episode. Testing can be via a urine test or a swab taken from any suspicious lesions in symptomatic patients. In those without symptoms a blood test can be taken to confirm infection – this is recommended for those patients who feel they may have been exposed to the herpes simplex virus. We offer all the above tests with 2 – 4 days depending on the test. Any patient testing positive should also be screened for other sexually transmitted infections as they can often co-exist. In females the symptoms include vaginal discharge, genital itching, pain on passing urine and lower abdominal pain. In male patients many cases can be picked up incidentally and there may be no symptoms. However, it may present in some males with penile discharge and pain on passing urine. Treatment is usually with an antibiotic called metronidazole – which can be given as a single high dose treatment or over the course of 1 week. Testing for trichomonas is usually performed by doing a urine test. Any patient testing positive should also be screened for other sexually transmitted infections as they can often co-exist. We offer high quality accurate testing with results available within 48 hours. The symptoms can be similar to other sexually transmitted infections, so testing specifically for mycoplasma is the only way to confirm infection. Most females are asymptomatic, in the remaining patients symptoms include vaginal discharge, pain on passing urine, pelvic pain, bleeding in between periods or after intercourse. In males symptoms include penile discharge and pain / stinging when passing urine. If left untreated it can cause pelvic inflammatory disease and infertility in females and testicular / epididymis infections in males. Treatment is usually with a course of antibiotics and retesting after treatment is recommended to ensure cure. Testing for mycoplasma genitalium can be done by a swab or urine test. Any patient testing positive should also be screened for other sexually transmitted infections as they can often co-exist. Risk factors include having a new sexual partner or multiple partners, not using barrier contraception (condoms), anal intercourse and oral sex. In addition to affecting the genital tract, gonorrhoea can affect the rectal and oropharyngeal tract. The infection is men is usually symptomatic, causing penile discharge and pain when passing urine. In females however approximately half of patients with the infection do not have symptoms. In the others symptoms can include pelvic pain, vaginal discharge, pain on passing urine and heavy periods / bleeding between periods. The treatment is most effective with a single dose of an antibiotic injection in combination with an oral antibiotic. All patients should have a repeat test after treatment to ensure clearance. Untreated gonorrhoea may lead to complications such as testicular infection and prostatitis in men. In females it may cause pelvic inflammatory disease, chronic pelvic pain and infertility. Testing for gonorrhoea may be done by either a swab or urine sample. Any patient testing positive should also be screened for other sexually transmitted infections as they can often co-exist. In most patients the infection is asymptomatic – meaning that an infection is present but doesn’t cause any symptoms. This can potentially cause complications in the long term (see below) and there is a risk of passing on the infection without knowing. In asymptomatic patients the only way on knowing you have the disease is by getting tested. Symptoms to look out for include: Women: vaginal discharge, pain on passing urine, pelvis pain Men: penile discharge, pain on passing urine, testicular swelling / pain You are more at risk of getting chlamydia if you don’t use barrier contraception (condoms), have a new partner or multiple partners. If Chlamydia is untreated it can potentially lead to pelvic infections and infertility in females, and testicular infections in males. The treatment is straight forward with antibiotics – the most common options being a single dose or a one week course. We would generally advise testing after treatment to ensure the infection has cleared. In addition, it is important to refrain from intercourse until a week after treatment if you receive the single dose, or for the duration of antibiotic treatment. Patients testing positive for chlamydia should also be screened for other sexually transmitted infections as they can often co-exist. Tests can be carried out by taking a swab sample or a urine test. We can offer high quality accurate testing with same day results. Book a completely confidential consultation with a highly experienced and sensitive GP to discuss your concerns or arrange testing. It is essentially reduced bone density which commonly occurs in females after the menopause. Oestrogen hormone has a role in protecting bone density, which declines after the menopause. Patients who have osteoporosis can suffer from significant, potentially life changing fractures. These can occur from a relatively minor impact which would not be expected to cause fractures in a healthy individual. The location can vary but commonly include the hip, shoulder and wrist. Also vertebral fractures can occur – leading to chronic back pain and loss of height. Risk factors for osteoporosis include: • Female Sex • Age • Premature menopause • Rheumatoid arthritis • Low BMI • Family history of hip fractures • Corticosteroid use • Smoking • Alcohol • Hyperthyroidism • Diabetes (type 1) Osteoporosis itself doesn’t cause any symptoms and most people find out they have it after a fracture. This is why screening is so important, the aim is to identify patients who have osteoporosis before a fracture occurs. Screening for osteoporosis is done by use of a risk assessment tool and a DEXA scan. Treatment can then be decided depending on the bone density (as measure by DEXA), the risk score and relevant history (whether you have had a previous fracture). Lifestyle modifications are important – reducing alcohol intake, avoiding smoking, weight bearing exercises and eating a well-balanced diet with calcium rich foods. The use of calcium and vitamin D supplements may be advised. Medication such as bisphosphonates can help reduce the breakdown of bone and reduce fractures, these are first line drug treatments in osteoporosis. Please contact us for further information. Due to aggressive forms which can metastasize there has been more proactive approach towards diagnosis and management. The main risk factors are: • Age • Family history • Ethnicity (more prevalent in the Afro-Caribbean population) Screening remains a controversial issue. A blood test called PSA (Prostate specific antigen) can be raised in patients with prostate cancer – but it is neither specific or sensitive enough to be used as a screening test. Consequently, the test may offer false reassurance or result in unnecessary investigations such as a prostate biopsy – which is an invasive test that can result in complications such as infection and bleeding. We recommend combining a PSA blood test with a detailed multiparametric prostate MRI using the latest state of the art 3T MRI scanners. A normal MRI scan can then avoid an unnecessary biopsy and furthermore any biopsies that are planned can be targeted to specific lesions. Please contact us for further information. The importance of screening cannot be underestimated - early detection and improved treatments have vastly improved outcomes. About a quarter of cases occur before the age of 50 so there is a case for early screening. Risk factors include: • Family history • Genetic mutations (BRCA1, BRCA2, TP53) • Hormone replacement therapy and the combined contraceptive pill (small increase in risk) The best screening tool is annual mammography after the age of 40, this uses x-rays to identify any suspicious breast lesions. However, we would consider both breast MRI and mammography in high risk individuals after the age of 30. Please contact us for further information. There are two main types – small cell and non-small cell. Survival rates are generally poor owing to diagnosis commonly occurring at a late stage. The main risk factors are: • Smoking • Passive smoking • Asbestos exposure • Radiation exposure Chest xrays are commonly used as an investigation but they are not particularly sensitive – they can miss many lung cancers. Low dose CT scanning is now recommended for patients considered to be at risk of developing lung cancer. We would offer such screening tests to those aged over 55 with a significant smoking history or other risk factors. Please contact us for further information. Book Appointment Online 0207 118 1816 8.30AM to 6PM 07863 112 919 Evenings and Weekends info@thegpcliniclondon-com.stackstaging.com ### Colorectal Cancer This is the third commonest cancer in the UK. Identifying it early can help save lives. Risk factors include: • Age • Family history • History of inflammatory bowel disease • Smoking • Obesity • Polyposis syndromes (FAP) • Dietary factors (excessive meat consumption, low fibre / vegetable intake) Making positive lifestyle and dietary changes can help lower your risk. Typical red flag symptoms / signs of colorectal cancer are: • Blood in the stools • Persistent change in bowel habit • Iron deficiency anaemia • Weight loss If you have any of these symptoms we would recommend you consult with a doctor urgently to get further investigations. However, these symptoms can often be a late sign of colorectal cancer, by the time they are present the disease may be at an advanced stage. The aim of screening for colorectal cancer is identify disease early enough so definitive treatment can be provided resulting in higher survival rates. Screening can also identify polyps in the bowel which have the potential to undergo malignant change. There are different forms of screening available: - Faecal occult blood (FOB) – this is a stool test which identifies blood in the stools even if it’s not visible to the naked eye. A positive result can potentially indicate bowel cancer, however there are other causes such as ulcers, haemorrhoids, diverticular disease etc. Anyone with a positive FOB would require further investigations, such as a colonoscopy or CT colonogram. We recommend annual FOB for low risk patients over the age of 50. We now offer the more accurate faecal immunochemical test (FIT) as standard for this type of test. - CT colonography - this test visualises the large bowel and can pick up tumours and polyps at an early stage. Even though a biopsy can't be taken, it is less invasive than a colonoscopy and serves as an excellent screening test. It has the added advantage of examining other structures in the abdomen including the liver, pancreas and kidneys. We recommend at least 5 yearly CT colonography for low risk patients. - Colonoscopy – this test directly visualises the large bowel and is usually performed under sedation. Although more invasive it’s a detailed test with the advantage of being able to take biopsies of any suspicious looking lesions. We recommend at least 10 yearly colonoscopy for low risk patients. Those at higher risk (family history, inflammatory bowel disease, presence of polyps etc) should be offered more frequent testing tailored to their individual case. Please contact us for further information. We can all lower our risk of cardiovascular disease and this is especially true of those at high risk. The main risk factors are: • Age • Male sex • Family History • Smoking • Diabetes • Raised Cholesterol • Hypertension • Obesity Obviously there is not much that can be done to change the first three items on the list! But by making positive lifestyle changes the others can be modified to reduce your risk. As part of the screening process we use all the information above to calculate a risk score for you – essentially the risk of having a heart attack or stroke in the next 10 years. We can then compare this risk to the score of a healthy person with the same age, sex and ethnicity as you. Furthermore, we can also use this data to calculate your heart age and compare this to your actual age. Your risk score would then further determine if we need to do further tests or recommend medications like statins to lower your risk. For example, a risk score of <10% is considered low risk. Between 10-20% we may recommend a CT Coronary Calcium scan to further asses your coronary arteries. A CT coronary calcium scan takes multiple images of your heart and looks at calcium build up in the coronary arteries. This in turn reflects whether is plaque building up in the coronary arteries which could cause narrowing leading to angina and heart attacks. CT coronary artery scores are graded and a tailored management plan put into place – such as aggressive risk factor reduction or referral to a cardiologist for further tests in cases of a high score. Please contact us for further information. The essence is to identify disease before a patient starts developing symptoms. If we can diagnose conditions early they are often more treatable – through lifestyle modification, medication or surgery – thereby leading to improved health outcomes. Many centres now perform whole body scans (MRI scans), but we believe this can potentially cause more harm than good. Firstly, some conditions are not picked up by a scan alone and therefore give false reassurance. In addition, there may be some findings on a scan which may not have caused any harm at all - leading to anxiety and unnecessary further tests which may be invasive and carry a risk of complications. Our approach is different. We offer a holistic and targeted health screening programme – this involves a thorough history – including discussing family history and risk factors. Followed by a full examination, urine tests, ECG and blood tests. Further tests are then offered depending on the outcome and these are focussed on some of the most common diseases. We not only want to identify disease but then work with you to manage and hopefully cure any condition we may find. Our health screening assessments are bespoke, evidence based and comprehensive for ultimate peace of mind. Well worth it!Briana Azar17:41 11 Sep 18My experiences at 25 Harley Street has always been exceptional. However, having Dr.Randy as my personal physician has been the best decision I've made for my health and wellbeing in London. Not only is he incredibly thorough, he is a doctor that looks at the whole picture when deciding what approach will work best for his patients. He has taken me off of so many over prescribed medications that I did not need and helped my body and mind to find the balance it so long had been searching for. He is capable, compassionate and versatile in his field and practice. Would highly recommend him!Sanchit Arora20:49 24 Jan 18I have had two great consultation sessions with Dr. Amarjit.
